Please bring all necessary items to your appointment:

  • Photo ID
  • Insurance card
  • A list of current medications
  • Any previous imaging scans such as X-rays (if available)

You should arrive 15 minutes early to complete any necessary paperwork.

During Your Visit

Your ÑÇÖÞ×ÔοÊÓƵ's appointment will take place in an exam room.  We understand your ÑÇÖÞ×ÔοÊÓƵ may be anxious. Your provider will take time to talk to you and your ÑÇÖÞ×ÔοÊÓƵ about questions or concerns. Your provider will ask about your ÑÇÖÞ×ÔοÊÓƵ's medical history and symptoms.
